PROVIDENT LIFE & ACCIDENT INSURANCE COMPANY

on behalf of all those who allege to have had claims for long-term disability benefits denied as part of an alleged "claims denial mill" operated by the largest disability in the United States. This wrongful pattern and practice was featured on CBS' 60 Minutes and NBC's Dateline. The suit asks for class certification, appointment of a system of Special Masters for re-evaluation of all claims denied since June 30, 1999 (many thousands), disgorgement of wrongfully taken profits and attorney's fees.
